This Privacy Policy describes how Infuse Pro LLC ("Infuse Pro," "we," "our," or "us") collects, uses, and shares information when you use the Infuse Pro mobile application and related services (the "Services"). Infuse Pro is a platform that connects patients with licensed mobile IV therapy providers in their area.
When you create an account, we collect your name, email address, phone number, date of birth, and password (stored as a one-way hash).
When you book a service, we collect the address where service will be delivered, the service you selected, any notes you provide, and the time of the appointment.
If your treating provider requires an intake form, we collect health information including but not limited to: medical history, current symptoms, allergies, medications, supplements, emergency contact information, and your electronic signature. This information is collected on behalf of the licensed provider treating you and is protected as protected health information (PHI) under HIPAA.
Photographs of IV sites or treatment areas may be taken by clinical staff during treatment and stored as part of your clinical record. These are accessible only to authorized clinical staff at the treating provider.
With your permission, we collect approximate or precise location information to help you find nearby providers and to share your service address with the provider you book with.
Payment information is processed by Stripe, our payment processor. We do not store your full credit card number; we store only the last 4 digits and card type for your records and the Stripe customer identifier.
Messages exchanged between you and your provider's staff during the period of an active booking are stored to enable the conversation and as part of the provider's clinical record.
We collect device identifiers, operating system, app version, IP address, access timestamps, crash logs, and basic usage analytics to operate, secure, and improve the Services.
We use the information we collect to:
We do not sell your personal information or PHI to third parties. We do not use your PHI for advertising.
When you book a service, your account information, booking details, and intake information are shared with the licensed mobile IV therapy provider treating you. The provider is the covered entity holding your medical record under HIPAA.
We share information with vendors who help us operate the Services, including Stripe (payment processing), SendGrid (transactional email), Firebase Cloud Messaging (push notifications), Amazon Web Services (hosting and storage), and Daily.co (when applicable, for telehealth video). These vendors are bound by contractual obligations to handle data appropriately.
We may disclose information if required by law, subpoena, court order, or other legal process, or to protect the rights, property, or safety of Infuse Pro, our users, or others.
In the event of a merger, acquisition, or sale of assets, your information may be transferred to the acquiring entity, subject to the protections of this Privacy Policy.
Health information collected by Infuse Pro on behalf of a licensed provider is protected health information (PHI) under HIPAA. The treating provider is the covered entity. Infuse Pro acts as a business associate to the providers on our marketplace and maintains the safeguards required under HIPAA, including administrative, physical, and technical safeguards. Requests to access, amend, or restrict the use of your medical records should be made to the treating provider directly.
We protect your information with industry-standard security measures including encryption in transit (TLS 1.2+), encryption at rest for sensitive data, access controls limiting access to authorized personnel, and audit logging of access to protected health information. No system is perfectly secure, but we work to protect your information at every stage.
We retain your account information as long as your account is active. Medical records and treatment history are held by the licensed provider treating you and retained for the period required by federal and state health record-retention laws (generally a minimum of 6 years, longer in some states). Booking history required for billing reconciliation, dispute resolution, and tax records is retained for 7 years. Aggregated, anonymized usage data may be retained indefinitely.
You have the right to:
Requests relating to your medical record should be made to the treating provider, who is the holder of those records under HIPAA.
The Services are intended for adults aged 18 and over. Minors may only be served as patients through a guardian account held by an adult. We do not knowingly collect information directly from children. If you believe a minor has provided information directly to us, please contact us so we can remove it.
The Services are operated from and intended for users in the United States. By using the Services, you consent to the transfer and processing of your information in the United States.
